FOREWORD Dr. Thomas Starzl and his colleagues have since their Colorado days spearheaded the recognition of the occurrence of posttransplant lymphoproliferative disorders and in a series of publications have contributed to their understanding and treatment. In this issue of Current Problems in Surgery, Drs. Nalesnik, Makowka, and Starzl have produced a superb and comprehensive monograph on this subject. The article is a tour de force of combined basic and clinical sciences, together with a concrete exposition of the various clinical factors involved from etiology, recognition, and treatment to ultimate outcome. As frightening as these disorders are, the heavy price they exact still turns out to be a tolerable price for the benefits of organ transplantation in the population at risk, particularly in view of the fact, as amply shown in these pages, that with appropriate therapy the tumors may be cured or made to regress, often without sacrifice of the transplanted organ. Diagnosis and management of posttransplant lymphoproliferative disorder in solid-organ transplant recipients.

The Epstein-Barr virus (EBV) has a pivotal pathophysiologic role in the development of most lymphoproliferative disorders that occur after solid-organ transplantation. The term "EBV-associated posttransplant lymphoproliferative disorder" (PTLD) includes all clinical syndromes of EBV-associated lymphoproliferation, ranging from uncomplicated posttransplant infectious mononucleosis to true malign...

Posttransplant Lymphoproliferative Disease after Pediatric Solid Organ Transplantation

Patients after solid organ transplantation (SOT) carry a substantially increased risk to develop malignant lymphomas. This is in part due to the immunosuppression required to maintain the function of the organ graft. Depending on the transplanted organ, up to 15% of pediatric transplant recipients acquire posttransplant lymphoproliferative disease (PTLD), and eventually 20% of those succumb to ...

Atypical Lymphoproliferative Diseases.

This review addresses the clinical presentation, pathology, and therapy of several uncommon lymphoid proliferations. Because these lymphoproliferations span the characteristics of reactive polymorphous proliferations to clonal malignant neoplasms, they are often difficult to diagnose and treat effectively. In Section I, Dr. Greiner describes the pathology of the spectrum of atypical lymphoid di...
